Designed to support the needs of patients and their families and facilitate a patient-centered approach to cancer care. The Oncology Nurse Navigator conducts comprehensive assessments of a patient’s individual needs, provides supportive care throughout the continuum of cancer treatment, connects patients to the health care team to facilitate patient-centered collaboration, identifies barriers to cancer care and works to eliminate them, and links patients to community resources.
